Flow to add row to a table when record is created or modified

I'm trying to create a flow that will add a row to a table when a record is created or modified. I've set up the trigger etc. but there's no way for me to map the values from the record to the new table. So when I run it, it shows succeeded but there are no new records in the table. This seems like it should be simple, but microsoft as usual makes it incredibly complicated. It also errors when I select columns to filter on, but that's another issue.

Please cross check table name in flow and the table which you are looking for.

Also, the environment of flow. may be flow created in some different in environment and you are looking in some other environment,

Based on your screenshots, there are three tables.

To be sure, the 'Deal Card Processes flow' is the table that triggers the flow.

On which table are you trying to create new records?

Application Status Histories? Agent Status Histories?
